Medicaid Recertification

You must go through the Medicaid recertification process every year and provide the evidence that Medicaid requests. Otherwise, there may be an interruption in your Platino coverage.

Step 1:

How can you identify when it is your turn to go through the Medicaid recertification process?

  • In the document that Medicaid gave you from your last evaluation, you can find the Medicaid eligibility period.
  • You can call Medicaid at 787-641-4224 to find out when your coverage expires.
  • Validate the recertification date by visiting: www.medicaid.pr.gov

Step 2:

Get ready for your recertification process. It's very simple!

Once you receive the letter from Medicaid for your annual recertification process, you must have a copy of the following documents for your appointment:

  1. Photo ID (license, passport, voter registration card)
  2. Health plan card
  3. Social Security card
  4. Medicare card (white, blue, and red)
  5. Social Security letter
  6. Proof of income
  1. Retirement, pension, employment, or Social Security check or stub
  2. Divorce certificate or decree
  3. W-2 form or Form 480
  4. Bank statement
  5. AAA and/or Luma receipt
  6. Evidence of U.S. Citizenship and/or legal or permanent residency

Step 3:

You can complete the recertification process through one of these options:

  • Complete and deliver the form to a local Medicaid office or mail it to P.O. Box 70184 San Juan, PR 00936-8184
  • Call Medicaid at 787-641-4224 Monday through Friday from 8 a.m. to 6 p.m. and request an in-person interview to complete the process.
  • Fill out the online form through the Medicaid Citizen Portal: www.medicaid.pr.gov
  • Call Medicaid at 787-274-5556 Monday through Friday from 8 a.m. to 1 p.m. for instructions on the documents to send via email.
  • Download the ASES VITAL mobile app and complete the process.

Are you no longer eligible for Medicaid?

At MCS Classicare we have different coverages with excellent benefits that do not require Medicaid eligibility. If you lose your Medicaid eligibility, contact us to support you during the process.
